ABSENTEE BALLOT REQUESTS FOR THE MAY 7, 2019 PRIMARY/SPECIAL ELECTION The Board of Elections is accepting written applications for Absentee ballots for the Special Election until Noon on Saturday, May 4, 2019. Absentee ballot request forms are available online at mcboe.org. A person may vote in-person at the Board of Elections office, for the Special Election, until 2:00 P.M. on Monday, May 6, 2019. A WRITTEN REQUEST FOR AN ABSENTEE BALLOT MUST INCLUDE: YOUR NAME VOTING RESIDENCE MAILING ADDRESS TO WHICH YOU WANT YOUR BALLOT MAILED YOUR BIRTH DATE ONE OF THE FOLLOWING Your OHIO driver's license or state ID number The last four (4) digits of your social security number Copy of current and valid photo identification, a military ID, a current utility bill, bank statement, government check, paycheck or other government document, other than a notice of voter registration mailed by a Board of Elections, that shows your name and address. DATE OF ELECTION YOU WISH TO VOTE A STATEMENT THAT YOU ARE A QUALIFIED ELECTOR YOUR SIGNATURE Requests must be mailed or delivered to: Montgomery County Board of Elections Absentee Department 451 W. 3rd St. Dayton, OH 45422 ALL COMPLETED ABSENTEE BALLOTS MUST BE RECEIVED AT THE BOARD OFFICE BY 7:30 P.M. ON ELECTION DAY IN ORDER TO BE COUNTED. THE ONLY EXCEPTIONS ARE: A VOTER OUTSIDE OF THE UNITED STATES, WHOSE BALLOT MUST BE POST-MARKED NO LATER THAN ELECTION DAY, AND RECEIVED WITHIN 10 DAYS AFTER THE ELECTION A VOTER FROM WITHIN THE UNITED STATES, WHOSE BALLOT MUST BE POST-MARKED NO LATER THAN THE DAY BEFORE ELECTION DAY AND RECEIVED WITHIN 10 DAYS AFTER THE ELECTION.
