Dow, LMT., via certified mail, return receipt requested, at her last known address of record, 3745 Capetian Ct., Apt 201, Fairborn, OH 45324-6656. The Notice was returned "return to sender, not deliverable as addressed, unable to forward". The notice states that the State Medical Board of Ohio proposes to approve her application for restoration of her certificate to practice massage therapy in Ohio provided she take and pass the Massage and Bodywork Licensing Examination (MBLEx). Ms. Dow is entitled to a hearing in this matter if such hearing is requested within thirty (30) days of the last date of publication of this notice. Ms. Dow may appear at such hearing in person, by her attorney, or by such other representative permitted to practice before this agency, or she may present her position, arguments or contentions in writing.
