City of Fairfield, Ohio Notice to Bidders But

CITY OF FAIRFIELD, OHIO NOTICE TO BIDDERS BUT / HAM SR 4 0.00 / 9.97 (PID 110415) Sealed proposals will be received by the Finance Department of the City of Fairfield, Ohio, until 3:00 p.m. Local Time, Monday, February 27, 2023, for furnishing all labor, materials, and equipment necessary to complete the project known as BUT / HAM SR 4 0.00 / 9.97 for the Department of Public Works at which time and place all bids will be publicly opened and read aloud. Detailed specifications and bid forms are on file in the Finance Office at the Fairfield Municipal Building, 5350 Pleasant Avenue, Fairfield, Ohio and online at Fairfield-city.org. Bids must be submitted on the forms in the Contract Documents and other conditions therein described must be met. Each bid must be enclosed in a sealed envelope, clearly marked "BUT / HAM SR 4 0.00 / 9.97 Project" on the face of the envelope and display the name and address of the bidder. A DBE goal of 7.00% has been established for this project. Cost of the plans and specifications is fifty dollars ($50) per set, non-refundable if obtained as a hardcopy, or no charge if obtained as an electronic version from the City's public website at https://www.fairfield-city.org/Bids.aspx. Each bid must be accompanied by the following completed documents / certifications: Bid Bond (Certified Check or Cashier's Check for ten percent (10%) of the amount of the bid submitted unless otherwise specified) Non-Collusion Affidavit Affidavit of Non-Delinquency of Personal Property Taxes The bidder hereby certifies that he has ...., has not ...., participated in a previous contract or subcontract subject to the equal opportunity clause, as required by Executive Orders 10925, 11114, or 11246, and that he has ...., has not ...., filed with the Joint Reporting Committee, the Director of the Office of Federal Contract Compliance, a Federal Government Contract Compliance, a Federal Government contracting or administering agency, or the former President's Committee on Equal Employment Opportunity, all reports due under the applicable filing requirements. The Bidder must circle the appropriate "has or has not" on the bid form. The bidder hereby certifies that all DBE commitments will be met. The bidder has submitted a DBE Utilization Plan. All Bidders shall submit a DBE Utilization Plan at the time of bid setting forth specific information demonstrating how the Bidder will achieve the DBE goal. By submitting a DBE Utilization Plan, the Bidder is affirming that they will be using the DBE firms identified in the Utilization Plan to meet the DBE contract goal. The DBE Utilization Plan shall be submitted with Formstack at time of bid submission. Any bids received without electronic submission of the DBE Utilization Plan at or before bid time, will be deemed unresponsive. Bidders shall submit their DBE Utilization Plans via: https://odot.formstack.com/ forms/dbe_copy Bid prices must be firm for a period of one hundred twenty (120) days following the bid opening date. Only pre-qualified contractors are eligible to submit bids for this PROJECT. Pre-qualification status must be in force at the time of bid, at the time of award, and through the life of the construction contract. For work types that ODOT does not pre-qualify, the LPA must still select a qualified contractor. Subcontractors are not subject to the pre-qualification requirement. The "prime" contractor must perform no less than 30 percent of the total original contract price. Each bidder is required to furnish with its proposal a Bid Guaranty and Contract Bond in accordance with Section 153.54 of the Ohio Revised Code. Bid security furnished in Bond form, shall be issued by a Surety Company or Corporation licensed in the State of Ohio to provide said surety. Each proposal must contain the full name of the party or parties submitting the proposal and all persons interested therein. Each bidder must submit evidence of its experiences on projects of similar size and complexity. The owner intends and requires that this project be completed no later than 180 calendar days from the award of the Notice to Proceed. All contractors and subcontractors involved with the project will to the extent practicable use Ohio products, materials, services, and labor in the implementation of their project. Additionally, contractor compliance with the equal opportunity requirements of Ohio Administrative Code Chapter 123 is required. Bidders must comply with the prevailing wage rates on Public Improvements in Butler County and the City of Fairfield, Ohio as determined by the Ohio Department of Commerce, Bureau of Wage and Hour Administration, 614.644.2239, and with the Federal prevailing wage rates as determined by the U.S. Department of Labor. The City of Fairfield reserves the right to reject any or all bids or to correct or waive irregularities in bids should it be deemed in the best interest of the City of Fairfield, Ohio to do so.
