LEGAL NOTICE REQUEST FOR PROPOSAL Pursuant to Ohio Revised Code Section 307.862, competitive sealed proposals ("proposals") will be received by the Board of County Commissioners, Clark County, Ohio, in the Office of the Clerk to the Board of County Commissioners, 3rd Floor, 3130 East Main Street, Springfield, Ohio 45503, UNTIL OCTOBER 8, 2026, at 10:00 A.M. EST for Near-Site Clinical Services. Request for proposal ("RFP") documents are on file at, and may be obtained from, the Board of Commissioners, 3rd Floor, Clark County, Ohio, in the Office of the Clerk to the Board of County Commissioners, 3130 East Main Street, Springfield, Ohio 45503, or by contacting Angela Clos at akclos@clarkcountyohio.gov. Please direct all questions to the Assistant County Administrator, in accordance with the RFP requirements. Each proposal shall contain the full name and address of each person or company submitting the proposal. One (1) original proposal and three (3) copies must be submitted to the Board of County Commissioners Office, 3130 East Main Street, 3rd Floor, Springfield, Ohio, 45503. They must be submitted NO LATER THAN OCTOBER 8, 2026, AT 10:00 A.M. EST. Proposals are to be clearly marked on the outside of the sealed envelope in the lower left-hand corner by name, as listed above. The County reserves the right to reject any and all proposals and to waive any defect in accordance with the RFP terms. Please note the County is exempt from all Federal, State, and Local Sales and/or Excise Taxes. Bidders must also submit the following one original and three copies: complete all questions in Exhibit C - Request for Proposal Response Near-Site Clinical Services; AND Non-Discrimination Provision, Personal Property Tax Statement, Non-Collusion Bid Affidavit, an Affidavit of Compliance with ORC 3517.13. The successful bidder must also submit a current Ohio BWC Certificate and Certificate of Liability Insurance that meets the County's insurance requirements: Requirements are included in the RFP package.
