NOTICE OF VOLUNTARY DISSOLUTION OF OHI HOSPICE, INC. TO: ALL CREDITORS AND CLAIMANTS OF OHI HOSPICE, INC. OHI Hospice, Inc., an Ohio nonprofit corporation, has dissolved. All claims shall be presented in writing to the corporation at 7575 Paragon Road, Dayton, OH 45459 and shall identify the claimant and contain sufficient information to reasonably inform the nonprofit corporation of the substance of the claim. The nonprofit corporation must receive the claim by no later than June 18, 2026. Claims received after such date will be barred.
