Public Notice of Closure Cambridge Home Health Care, Inc. d/b/a Home Care by Black Stone of Kettering, OH will cease operations on 10/30/2026 and will no longer participate in the Medicare Program as provided under Title XVIII of the Social Security Act. The Medicare provider agreement between Cambridge Home Health Care, Inc. d/b/a Home Care by Black Stone and the federal Department of Health and Hospitals will terminate effective 10/30/2026. The Medicare program will not make payment for services furnished to any patients who may be admitted on or after 10/30/2026. However, Medicare payment is available for services provided under a plan of care established before the effective date of termination for a period of thirty (30) days following closure.
